W-9 — How to Use Form W-9 for Tax Reporting — IRS Use Form W-7, Application for Indirect Tax Withholding Form, to apply for an IRS ITIN. This form does not provide information about the  person being provided with the ITIN. It simply requires a copy of the IRS Form 1099-R. The form will be issued to taxpayers when the payment is accepted or  made and any tax owed on the payment is remitted. You typically obtain the 1099-R from the business where the payment is made or from the business where you and the dependent taxpayer have an account. If your spouse is a U.S. citizen, all documentation listed above will be accepted.
