Amount of payroll taxes paid to employees, including any amounts imposed on employers. Amounts paid by the employee for the employee's noncovered elective employee health plan. Amounts paid to the employee for any other health plan, excluding self-only coverage. Amounts paid to an employee by an employer for the purchase of items, including meals and lodging. See IRC Section 4980H for a discussion of the allowable deductions for food and lodging for non-discretionary items of clothing, household goods and housing. Allowable net income from self-only coverage. Any other itemized deductions, including the following items of income for the calendar year, and the additional allowable deductions for the calendar year:  Amounts for any non-cash itemized deductions, excluding amounts for the following items:  Tax withholding (Social Security and Medicare); and Amounts paid to any federal, state, or local governmental entity. See IRC Section 4975(a).
