Form WG-10 (Paycheck Allowance Statement). Provide the following details of the business. The total pay for the calendar year must not exceed 600. In addition to the 2,500 in wages, the following amounts must be deducted: Amount of wages paid to the employee for health insurance (excluding self-only or family coverage), other than a health tax-exempt employee health plan (such as an SEP or CPP) Amount paid to the employees in accordance with IRC § 6421 Amounts of wages paid for the calendar year other than wages for the employee's self-only coverage on which Medicare tax is not paid Amounts paid to the employees by a trade or business and which are excluded from gross income under IRC Section 401(a)(7) Amount of wages paid to the employee for other health coverage, excluding self-only coverage. Amount of payroll taxes paid to employees, including any amounts imposed on employers. Amounts paid by the employee for the employee's noncovered elective employee health plan. Amounts paid to the employee for any other health plan, excluding self-only coverage. Amounts paid to an employee by an employer for the purchase of items, including meals and lodging. Allowable net income from self-only coverage. Any other itemized deductions, including the following items of income for the calendar year, and the additional allowable deductions for the calendar year: Amounts for any non-cash itemized deductions, excluding amounts for the following items: Tax withholding (Social Security and Medicare); and Amounts paid to any federal, state, or local governmental entity.
