Grand Prairie Texas Form W-9: What You Should Know

All completed questionnaires will be included in the final report to the City of Grand Prairie to be used in funding development of our master plan and zoning process. W-9 (Form W-9, Wage and Tax Statement) is a paper written declaration stating your source of income for the last year; and declaring that the information given is true within the meaning of Section 6103(a) (1) of the Internal Revenue Code, in the case tax statement to be filed with the Internal Revenue Service, and to the satisfaction of the Department of the Treasury, if the information given is not true. If more than one person uses the statement, it must have as its signature the Chief Financial Officer and each director or officer of the school or organization for which the statement was prepared. (W-6) The W-6 form is available for public inspection. You may also print or read it here: 1) Complete this information and submit it to your principal and/or the Director. 2) Attach forms for the teachers. This can be done by e-mailing them to me at Kris [at] MCG.k12.GA.us. There are forms for each district available. For example, you would send this, the teacher W-8 form, and the school W-6 form. The W-9 is used for a student or student group of 6 or more people.
