W-9 (Form W-9, Wage and Tax Statement) is a paper written declaration stating your source of income for the last year; and declaring that the information given is true within the meaning of Section 6103(a) (1) of the Internal Revenue Code, in the case tax statement to be filed with the Internal Revenue Service, and to the satisfaction of the Department of the Treasury, if the information given is not true. If more than one person uses the statement, it must have as its signature the Chief Financial Officer and each director or officer of the school or organization for which the statement was prepared.
