Your information and tax return is protected! • If you are having difficulty downloading your Form 1040A (Form 1040), please visit this website and provide the following:  Identification of the taxpayer • The name of your state tax agency or office; • The date of each tax return; • The taxpayer identification number, and the identification number on your city tax form (see sample form). Each individual has 2 numbers available, “A” and “B.” • Your first number on your form is the taxpayer identification number, and the second is the identification number on your city tax form for your name, address and social security number. Form W-4 is a copy of your tax return and is the information you will need to get your state tax return! When filing your tax form, you should sign your name and send it to the city clerk. Your Tax Return Information Form will be mailed to the taxpayer if you pay your taxes on time and keep all of your tax return information required in the law.
