Provide information under “Full Name” of a person authorized to act on behalf of the taxpayer. Do not send to the IRS. 2 Social Security Number. Enter the Social Security number of the person authorized to act on behalf of the taxpayer (e.g. your employer or your parent who is a dependent). This may be the same SSN (number) used in IRS form 1040 line 41, “Name Identification.” If you are required to fill out the SSN of your parent to file an IN on your tax return, the parent's SSN may be shown on Form W-2 shown on line 20 of your tax return and on any Form 1040A, “Annex to U.S. Fax or Postal Tax Return or Claim for Refund of Excise and/or GST Taxes,” showing a child not filed. 3 Form W-9. The “TIN” of the person to whom Form W-9 is issued by the IRS is the same for all individuals (e.g., parent, grandparent, child, etc.) and must be entered on line 11 of this form. The TIN is your taxpayer identification number entered on line 3 under “Full Name” of the person who must sign on it. The person to whom your taxes are owed is your “employee.” The number on Line 10 under “Employee Identification Number” that you provided on your income tax return is the employer identification number. If you are an employee who receives a Form W-2 and Form W-9 in the mail, the address on the Form W-2 and Form W-9 must match the address on your tax return. This does not apply to Form 1040, “Annual Return of Income or Employee Filing Status.” The Form W-9 must match the tax return address (e.g., “Employee of ___________”) on the first page, the address shown on line 11 of Form W-2 on the second page, and the address you enter in box 11 on the Form 1040. 4 Date. Enter the date you started working for the employer (or as a dependent) in box 14 of each Form W-2. 5 Social Security Number. Enter the Social Security account number entered on Form W-2. Do NOT send this to the IRS. 6 Employer Identification. Enter an employer and TIN that are not duplicates.
