You should indicate on the form whether any service is to be rendered for a fee or other thing of value, such as a subscription. If you are a sole proprietorship or partnership and have authorized the provider for the billing and collection of fees or other thing-of-value, you will need to verify with the provider that this is permitted. Be sure to get your own copy of any information provided by the provider and keep a copy for your records. You will need to provide a signed copy of your completed Form W-9 with the following information in the space provided by the employer for the name and address the TIN of the person requesting payment.
