I also authorize the bank/financial institution to deduct the cost of the funds from the monies to be deposited and the costs of the transfer from the URGE to the bank/financial institution. I understand the funds deposited will be held at the account/financial institution where deposited and I agree to the terms specified during the direct deposits' procedure. To receive a notice of the deposit, you must provide a photo ID and complete and sign the Form W9 or a statement certifying that the statement is true and correct. I understand that, if the URGE is unable to complete the transaction because it has insufficient funds to do so, I authorize the URGE to charge me interest. I understand that the URGE is not liable for late disbursements. The deposit must be made no later than 10 days after the date of the written agreement described above.
